Lisnaskea Supported Living Services provide care and support to adults with mental ill health and dementia-related needs across supported accommodation and floating support models in the Fermanagh and Tyrone areas. The service is funded by local Health and Social Care Trusts and the Supporting People programme.
Supported Living (Flat Cluster)
Provides supported living to 11 adults with mental ill health. Each person has their own self-contained flat with access to 24/7 staff support. The service is located close to local amenities and is suitable for individuals who benefit from a structured, supported environment. Referrals are made via the NIHE Complex Needs List and Community Mental Health Teams.
Floating Support – Mental Health
Provides short-term housing-related support to up to 20 adults with mental ill health living in their own homes in the Fermanagh area. The service aims to maximise independence, improve tenancy sustainment, and enhance overall wellbeing. Referrals are accepted from statutory professionals.
Floating Support – Older People with Dementia
Provides short-term floating support for to up to two years for 35 adults aged 55+ with dementia-related needs in the Fermanagh and Tyrone areas. The service is available to individuals with a diagnosis, those undergoing assessment, or people experiencing memory difficulties. Support is designed to promote independent living and may last from a few weeks to up to two years. Referrals are accepted from statutory professionals and self-referrals. This service is funded by the Supporting People programme.
