Nominate a person supported by Praxis Care who has excelled in last 12 months

Do you know someone who has gone above and beyond to reach their goals? Has someone’s personal progress blown your mind? Shout about it by nominating them in the 2022 Celebrating Success Awards!

The deadline for nominations is Tuesday, 20 September at 5 pm. All nominations should be submitted via the website form.

Anyone can nominate someone they know who is supported by Praxis Care who they feel has excelled in their own way in the last 12 months. All shortlisted nominees will receive cash prizes up to £200/€200.

Categories include:

• The Co-Production Award for someone who made a meaningful difference by sharing their ideas and views
• The Health and Wellbeing Award for someone who has achieved, maintained and promoted a healthy lifestyle
• The Community Involvement/Active Citizen Award for making a significant contribution to their local community
• The Person of the Year Award for exceptional achievements towards their goals
• The Praxis Talent Award will go to someone who has shown their talents and skills in anything from the arts to horticulture to the kitchen…the options are endless

It’s easy to nominate! Nominations are up to 200 words and any supporting files (videos, photos, etc).
